The NTSB Probable Cause<\/em> cited:<\/h3>\n\n\n\n
\n

\u2026.the flight crew\u2019s mismanagement of the airplane\u2019s descent during the visual approach, the PF\u2019s unintended deactivation of automatic airspeed control, the flight crew\u2019s inadequate monitoring of airspeed, and the flight crew\u2019s delayed execution of a go-around after they became aware that the airplane was below acceptable glidepath and airspeed tolerances.<\/p>\n\n\n\n

Contributing to the accident were<\/em><\/p>\n\n\n\n

(1) the complexities of the autothrottle and autopilot flight director systems that were inadequately described in Boeing\u2019s documentation and Asiana\u2019s pilot training, which increased the likelihood of mode error;<\/p>\n\n\n\n

(2) the flight crew\u2019s nonstandard communication and coordination regarding the use of the autothrottle and autopilot flight director systems;<\/p>\n\n\n\n

(3) the PF\u2019s inadequate training on the planning and execution of visual approaches;<\/p>\n\n\n\n

(4) the PM\/instructor pilot\u2019s inadequate supervision of the PF; and<\/p>\n\n\n\n

(5) flight crew fatigue, which likely degraded their performance.<\/p>\n<\/blockquote>\n\n\n\n
